APPLICATION FORM

I wish to apply for membership of the Malaga and Districts Business Association, and agree to abide by the constitution and rules of the Association. The current joining fee is $110.00 inc GST plus a pro-rata amount for the number of months from the date of joining up to 30 June of the current year. Second year $192.50 inc GST.

Membership fees fall due on 1 July each year.

Please complete the application form online so that it is legible. Then print and post with your cheque or fax if paying by credit card. Depending on the version of Acrobat Reader you may be able to save the form with the data filled in and also email the filled in form by using the Email button on the bottom of the form.
